What to bring to the appointment
- Kachemak Bay Animal Hospital's role is to complete the travel certificate, administer and document vaccines, microchip, and other requirements; however, it is your responsibility to understand the requirements and make sure they are met within the required time frame. Review the requirements for pet travel to your destination state or country, even if you are just passing through. Certain destinations have specific requirements which can take over 3 months to meet before a health certificate can be issued.
- Health certificates are valid for 10 days by air travel and includes the return trip for up to 30 days. The certificates are valid for 30 days by land, so make sure to plan your health certificate appointment and travel date accordingly.
- For International Travel (Outside of Mainland U.S., Hawaii, Canada and Mexico), please call to schedule a consult/office visit to discuss IF we are able to assist you with your travel plans and go over starting costs

Contact your airline(s)
- Some airline(s) have additional requirements, including size and type of carrier, dog breeds allowed, etc, specific forms that may need to be completed.
What to bring to the appointment
- Fill out our pet travel form by following this link at least 48 hours prior to the appointment
- If you are unable to fill out our form prior to the appointment, please bring:
- The full name, current address and phone number of the person traveling with the animal(s)
- The full name, current address and phone number of the person, hotel, or organization receiving the animal at the final destination (if different)
- The exact date your pet will be traveling, method of transportation and port of entry
- All previous records from other veterinary clinics, especially vaccine records
- Many health certificates need a signed rabies certificate and the date of microchip implantation. Please bring these forms and dates with you to your pet's appointment.
